  • CHHA-Certified HomeHealth Aide

    We are looking for dedicated CHHAs who want to provide quality care to home based patients. You will work one-on-one with the patient and truly impact their life as well as their family's.

    The job is consistent, long-term, and can work with a variety of different schedules. The CNA is responsible for providing direct patient care under the supervision of a registered nurse.

    A Home Health Aide (HHA) is a crucial role in the healthcare system, providing essential care and support to patients in their own homes.

    Responsibilities:

    Provides direct patient care as defined in California Nurse Practice Act.

    Assist clients in their daily personal tasks, such as grooming patient, bathing or dressing.

    Perform housekeeping tasks, such as laundry, washing dishes, and vacuuming. Help to organize a client's schedule and plan appointments.

    Arrange transportation to doctors' offices or other outings.

    Handling basic logistics, including paying bills and scheduling appointments.

    A Certified Nurse Assistant (CNA) is a Vital asset in healthcare providing essential care and support to patients in their own homes.

    Responsibilities:

    Provide basic patient care and treatment collaborated with registered nurses patient’s specific plan of care. Educate and update patients and families on medical treatments. accurate and timely documentation skills are necessary.

    Ability to provide comfort and support to patients, assisting with personal hygiene and other activities of daily living, therapeutic activities, and alternative activities to prevent restraint.

  • LVN-Licensed Vocational Nurse

    The job is consistent, long-term, and can work with a variety of different schedules. The LVN is responsible for providing direct patient care under the supervision of a registered nurse.

    A Licensed Vocational Nurse (LVN) is a crucial role in the healthcare providing essential care and support to patients in their own homes.

    Responsibilities:

    Provides direct patient care as defined in California Nurse Practice Act.

    Implements plan of care initiated by the registered nurse.

    Provides accurate and timely documentation consistent with the plan of care and prepares clinical notes.

    Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.

    Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse (RN) and/or Clinical Supervisor/Nursing Supervisor.

    Uses equipment and supplies effectively and efficiently.

    Participates in personal and professional growth and development.

    Performs other duties as assigned by the registered nurse (RN).

    Participates in the ongoing interdisciplinary assessment of the patient.

    Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver (s).

    Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.

    Participates in the organization’s quality assessment and performance improvement (QAPI) program.

    Participates in organization-sponsored in-service training.

  • RN-Registered Nurse

    We are looking for dedicated RNs who want to provide quality care to home based patients. You will work one-on-one with the patient and truly impact their life as well as their family's.

    The job is consistent, long-term, and can work with a variety of different schedules. The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience.

    A Registered Nurse (RN) is a crucial role in healthcare providing essential care and support to patients in their own homes.

    Responsibilities:

    Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(s).

    Regularly re-evaluates patient nursing needs.

    Initiates the plan of care and makes necessary revisions as patient status and needs change.

    Uses health assessment data to determine nursing diagnosis.

    Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.

    Initiates appropriate preventive and rehabilitative nursing procedures.

    Administers medications and treatments as prescribed by the physician.

    Counsels the patient and family in meeting nursing and related needs.

    Provides health care instructions to the patient as appropriate per assessment and plan of care.

    Identifies discharge planning needs as part of the care plan development and implements before discharge of the patient.

    Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility for coordinating patient care for assigned caseload.

    Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.

    Participates in the organization’s quality assessment and performance improvement (QAPI) program.

    Participates in organization-sponsored in-service training.

To be homebound means:

■ You have trouble leaving your home without help (like using a cane, wheelchair, walker, or crutches; special transportation; or help from another person) because of an illness or injury, or leaving your home isn’t recommended because of your condition.
